Instructions for Signing On
- Only national, state or local organizations, including institutions, universities, or separate discrete schools can be signed on. Individuals cannot be signed on.
- State and local organizations should identify the state, city or county as follows:
- state organizations – if not already integral to the organization name, e.g., Virginia Health Promotion Association, add the abbreviated state name, e.g., Health Promotion Association, VA
- city or county organizations – same as for state organizations, except cities need to be spelled out. The word county can be abbreviated, e.g., Health Promotion Association, Smith, CO
- DO NOT USE ACRONYMS – fully spell out the name of the organization, e.g., do not list NHPA for National Health Promotion Association – spell it all out.
- After signing, check to see that you have correctly spelled the organization, institution, university, or school name.
- Those signing on should have authority to do so, direct or conferred.
